Go to www.mymaths.co.uk, click on the “Subscribe” tab located on the navigation bar at the top of the page, then select “Primary”, “Secondary” or “Post 16”, click the button located at the bottom right corner of the page that says “Application Form”, fill out the regitration form, and click on “Print this page”.
